Uncertainty regarding treatment and prognosis in patients with chronic kidney disease: scoping review

Abstract
Objective. To identify the presence and magnitude of uncertainty regarding treatment and prognosis in patients with chronic kidney disease. Method. A scoping review was conducted in databases such as PubMed, LILACS, Redalyc, Google Scholar, and EBSCO. The descriptors used were “Renal Insufficiency, Chronic,” “Uncertainty,” “Prognosis,” “Treatment,” and “Therapeutics.” Original and review articles in Spanish and English published between 2020 and 2025 were included. Of the 1,161 articles found, eight met the inclusion criteria. Results. A total of 1,161 articles were identified; after screening titles and abstracts, 39 were selected, and following full-text review, 31 were excluded due to lack of access or failure to meet the criteria, resulting in eight documents included in this review. Conclusion. Uncertainty was found regarding the treatment and prognosis of chronic kidney disease; however, there remains a gap concerning the extent to which this uncertainty occurs. Studies primarily focused on terminal-stage patients and used qualitative approaches. It was identified that uncertainty impacts decision-making, treatment adherence, and overall patient well-being. It is suggested to assess uncertainty quantitatively across all stages of the disease and to incorporate nursing theoretical frameworks, such as Merle Mishel’s Uncertainty in Illness Theory, in addressing this phenomenon.
